Participation Waiver & Release

Please read this waiver carefully before signing. I am the parent or legal guardian of the player being registered, and I give permission for the player to participate in Muslim Youths FC training sessions, games, events, warm-ups, and related football activities during the Summer 2026 season.

I understand that football is a physically demanding sport. Participation may involve running, sprinting, turning, jumping, heading, kicking, sliding, contact with other players, contact with the ball, contact with the ground or field equipment, and changing weather or field conditions.

I understand that risks may include, but are not limited to, falls, collisions, cuts, bruises, sprains, strains, muscle injuries, joint injuries, dislocations, broken bones, concussions, heat illness, dehydration, communicable illness, paralysis, death, and other physical, mental, or emotional injury. Serious injury is uncommon, but it can occur even when reasonable care is taken.

I confirm that the player is physically able to participate to the best of my knowledge. I agree that the player should not participate while sick, injured, medically restricted, or experiencing symptoms that could put the player or others at risk. I agree to update MYFC promptly if medical, emergency contact, or registration information changes.

I agree that the player must follow coach instructions, referee instructions, field rules, facility rules, fair-play expectations, and safety directions at all MYFC activities. I understand that MYFC may remove a player from an activity if behaviour creates a safety concern for that player, another player, staff, volunteers, spectators, or facility users.

I understand that MYFC activities take place at rented or partner facilities, including indoor and outdoor venues. I agree that the player and accompanying family members must respect the rules of each premises and activity, remain in permitted areas, treat others respectfully, accommodate other facility users, and leave the premises if asked by MYFC staff, facility staff, or authorized representatives.

I understand that parents and guardians remain responsible for supervising their child before and after scheduled activities, during arrival and pickup, and whenever the child is outside active coach-led play. Parents and guardians are expected to monitor younger children and siblings who attend as spectators.

If urgent care is needed and I cannot be reached immediately, I authorize MYFC coaches, staff, volunteers, or organizers to seek reasonable emergency medical assistance for the player. I understand that I am responsible for any medical, ambulance, or related costs not covered by public or private insurance.

I understand that facilities may use security cameras or other monitoring systems for safety, security, or operational purposes. MYFC does not control every facility monitoring system, and I understand that facility operators may keep or share footage according to their own policies and legal obligations.

I understand that families may take photos or videos at MYFC activities for personal use. I agree to make reasonable efforts not to photograph or record children or families outside my own group in a way that is intrusive or disrespectful. MYFC media permission is handled separately in the registration acknowledgements below.

To the fullest extent permitted by Ontario law, I accept the risks of participation and agree to GIVE UP MY RIGHT TO SUE Muslim Youths FC, its organizers, coaches, volunteers, staff, partners, sponsors, associated organizations, and facility providers for any claim, demand, damage, expense, physical or emotional injury, paralysis, death, or loss that I, the player, my family, heirs, estate, or personal representatives may suffer in connection with MYFC activities or facilities.

This release includes claims arising from ordinary negligence, failure to warn or protect from risks, breach of contract, breach of duty of care, facility conditions, equipment, supervision, or participation in football activities, except to the extent a claim cannot legally be waived under Ontario law.

I understand that this waiver is intended to apply to all MYFC Summer 2026 activities, including games, training, warm-ups, team activities, arrival, pickup, and use of related facilities. I understand that this waiver is governed by the laws of Ontario and is intended to be interpreted as broadly as permitted by law.

By signing below, I confirm that I have read this waiver, understand it, and agree to it on behalf of myself and the player being registered.
